The GIM Heart Failure Fellowship at UHN is a 1-year fellowship designed to enhance heart failure knowledge and skills for internal medicine specialists in the care of patients with heart failure syndrome. There will be a mix of outpatient clinics and inpatient heart failure consultations where fellows will:
Learn the interpretation and application of diagnostic procedures.
Be proficient in initiation and titration of pharmacological therapy.
Establish longitudinal relationships with patients with heart failure syndrome that spans across the disease spectrum (ambulatory - inpatient and transitions of care).
Have an approach to the care of patients with advanced heart failure in the community (referral pathways to subspecialty services).
