Description:

The Schulich Heart Program is pleased to offer a one-year multi-modality clinical imaging fellowship.  The major focus of fellow will be echocardiography, with the goal of achieving a high level of clinical expertise in all applications of this modality. Qualification for Level III certification in Echocardiography, and completion of the National Board of Echocardiography examination, which takes place in July each year, are worthy goals. In addition there will be exposure to cardiac MRI and CT.
It is possible that the fellow will be able to achieve level III certification in MRI and CT depending on their interest, background experience,  and time commitment to this aspect of the fellowship.

Primary supervision of the imaging fellow will be by Drs. Joyner, Ahmed, Hansen, Poon and Roifman (echocardiography) and Dr. Anna Zavodni- Medical Imaging (MRI and CT). Drs. Gilbert, Morgan, Poon and Lau will also contribute to supervision of echocardiography. Primary supervision of research will be by Dr. Graham Wright.

Clinical Responsibilities:

The fellow will be expected to be involved in all aspects of cardiac imaging-related procedures, including:

  • Communicating with referring physicians regarding appropriateness and suitability for imaging tests
  • Triage of cases and thoughtful assignment of resources
  • Performing and interpreting surface echocardiograms, including use of intravenous saline and echocardiographic contrast (it is expected that the fellow will demonstrate and maintain excellent hands on echo skills)
  • Performing and interpreting trans-esophageal echocardiograms, including involvement with intra-operative TEE and 3D echocardiography
  • Post-procedure documentation and orders after TEE
  • Supervision and interpretation of  exercise stress echocardiography
  • Supervision and  interpretation of  cardiac MRI and CT studies (one day/week)
  • Communication of results to referring physician or service

 

It is not expected that fellow be involved with every task for every patient. However, it is the joint responsibility of the attending staff, imaging technologists, and fellow to ensure that educational opportunities involving interesting, rare, or unusual cases not be missed.

Fellows will be expected to be involved with interpretation and/or scanning of a large volume of echo studies daily, including all interesting/rare/unusual cases that pass through the lab. The fellow will be expected to take part regularly in supervision and interpretation of exercise stress echo. The fellow should be involved in most TEEs that pass through the lab.

It is expected that the imaging fellow may be asked to help occasionally with clinical care of CCU or Cardiology ward patients.

The fellow may be asked to participate in a TEE on call schedule shared with Anesthesia. No other call responsibility is mandated, although the option of after-hours involvement with interesting cases is encouraged if it is felt to be of genuine educational value, and not an onerous burden.

Research:

There is the option of being involved in a research project depending on the  needs and interest of the fellow.  The supervisor for this component of the fellowship is Dr. Graham Wright, research director for the Schulich Heart Program. There is the potential to spend one day per week on research. 

 
Rounds and Teaching:

Teaching is a key mandate of the Sunnybrook non-invasive lab, and an important component of the cardiac MRI and CT programs. The fellow is expected both to benefit from, and contribute to, an excellent teaching environment. Cooperation with core cardiology trainees, elective trainees, and the other Imaging Fellow– especially in situations of limited educational resources – is essential in maintaining a productive atmosphere.

Teaching of cardiology residents is an important mission of the non-invasive cardiac laboratories and the fellow is expected to assist in this process. “To teach is to learn” – and it is hoped that the fellow will contribute to this mission. In return, the fellow will receive regular, formal and informal teaching in all aspects of clinical imaging and in conjunction with research efforts.

The fellow will be responsible for presenting and leading non-invasive imaging rounds on a regular basis. Presentation at cardiology grand rounds and cardiology research rounds is strongly encouraged. Presentation of educational and quality-assurance rounds within the Echocardiography Lab is also a key component of the fellowship.

Fellows will be provided with a pager, lab coats and scrub suits, a comfortable workspace (shared office), and access to a computer with internet access. Funding support to attend at conferences be provided if the fellow is presenting original research. Vacation allowance consists of four weeks per year.

 

Evaluation:
In addition to ongoing, informal bi-directional feedback, the fellow will meet with the noninvasive cardiac laboratory director every six months with both parties providing oral evaluation. A written evaluation will be completed after 12 months. On completion of the fellowship program, a certificate will be issued by the Hospital and the University.
